Desafios da Segurança do Paciente na Atenção Primária à Saúde no Brasil: Revisão de Escopo

Resumo

A segurança do paciente é componente essencial da qualidade assistencial e enfrenta desafios particulares na atenção primária à saúde (APS), porta de entrada do Sistema Único de Saúde (SUS). Realizou-se uma revisão de escopo nas bases LILACS, MEDLINE, IBECS, BDENF e CINAHL, além das bibliotecas Cochrane, SciELO, PubMed e Web of Science. Foram incluídos artigos originais publicados entre 2013 e 2022, conduzidos no Brasil, envolvendo profissionais de saúde ou gestores da APS. A seleção seguiu as recomendações do PRISMA-ScR. Foram identificados 1.378 registros, resultando em 19 estudos incluídos. A análise revelou quatro eixos principais: (1) desafios dos profissionais de saúde, como falhas de comunicação, sobrecarga de trabalho e insuficiência de protocolos; (2) desafios da gestão, incluindo falta de infraestrutura, recursos e apoio da liderança; (3) desafios relacionados a usuários e famílias, especialmente a não adesão ao tratamento e fragilidades de vínculo; e (4) recursos potencializadores, como educação permanente, comunicação interprofissional e participação dos usuários. Os achados evidenciam barreiras estruturais, organizacionais e relacionais que comprometem a segurança do paciente na APS brasileira. O fortalecimento da cultura de segurança e a incorporação de práticas de gestão participativa podem subsidiar políticas públicas voltadas para a redução de eventos adversos e a qualificação do cuidado em saúde.
