Breast cancer is associated with increased cardiovascular risk, as cardiovascular diseases are the leading cause of non-oncological death in this population. This Integrative Review analyzed 46 articles selected from PubMed/MEDLINE, SciELO, and Europe PMC databases, aiming to synthesize evidence on the pathophysiological mechanisms, cardiotoxic agents, and cardiovascular prevention and monitoring strategies in breast cancer patients. The findings showed that anthracyclines, trastuzumab, aromatase inhibitors, CDK 4/6 inhibitors, and thoracic radiotherapy are the main therapies inducing cardiotoxicity, and are potentiated by risk factors such as obesity, hypertension, metabolic syndrome, and induced menopause. Cardiac biomarkers and global longitudinal strain demonstrated superiority in the early detection of subclinical cardiac dysfunction. Therefore, the integration between cardiology and oncology, with individualized risk stratification and longitudinal follow-up, are essential for reducing cardiovascular morbidity and mortality among breast cancer patients and survivors.
